I)   Note: Must Reduce Dose or Increase Lockout Interval and Eliminate
     Continuous Infusion for Patients having any of the following
          Conditions;
              A) Greater than 60yrs old;
              B) Morbid Obesity
              C) Pulmonary Impairment
              D) Hepatic Impairment
              E) Renal Impairment
 II)  Usual Lockout Range is 6 to 20 minutes
 III) Meperidine Cannot be used as a Continuous Infusion
 IV) An optional Four Hr Dose Limit (usually 15-30mg) may be added to the PCA prescription
