- Adult: 0.5-2gm q8hr IM/IV; t1/2=2h; Max dose: 6 gm/24hr;
- surgical prophylaxis: 1gm IM/IV 30-60min before surgery and 500mg - 1gm q6-8hr afterwards for 24hr;
- Peds: ( > 4 yrs): 100 mg/kg/24 hr
- Caution:
- w/ allergy to Penicillin and contraindicated with allergry to cephalosporin;
- Dosing Regimens for Patients with Renal Insufficiency: (Dose for 70 kg Adult (gm/dosing interval in hours)):
- 70-80 % of dose will be excreted in urine;
- CrCl > 80 1 q 8, CrCl 50-79 1 q 8, CrCl 30-49 1 q 12, CrCl 10-29 0.5 q 12
- w/ osteomyelitis:
