Fractures and Dislocations of the Proximal Fibula


- See:
       - Lateral Collateral Ligament
       - Maisonneuve Fracture

- Discussion:
      - occur most often in association w/ proximal tibial fractures, esp w/ split compression, bicondylar, and subcondlylar fractures;
      - look for injury to peroneal nerve, anterior tibial artery, biceps tendon, & lateral collateral ligament;
      - mech is usually a varus force;

- Avulsion Off the Fibular Styloid:
      - exception is an avulsion of the fibular styloid with its attached biceps tendon and fibular collateral ligament;
      - after fibular avulsion, it is important to test for peroneal nerve function &, if deficit is identified, nerve should be explored;
      - knee instability is another reason to fixed theses fractures;

- Dislocation of the Head of the Fibula:
      - initially attempt treatment w/ closed reduction and casting in extension;
      - Anterolateral dislocation of the head of the fibula in sports.
      - Isolated anterior dislocation of the proximal tibiofibular joint.
