FVFG: Insurance and Disability

- Insurance: Your insurance policy is a contract between you and your insurance company. It is most important that you understand its provisions. Review your policy and become familiar with it's benefits and provisions. In general we file insurance as a courtesy to the patient, but will provide information you need for filing. Any dispute on payment is between the patient and insurance company. If you have any questions about the coverage you should contact the insurance specialist of your employer's insurance company, questions regarding the filing of claims or the responsibility of payment can be discussed with the office personnel handling those claims. The CPT codes are 20955 - bone graft with microvascular anastomoses, fibula and 27170 - core decompression with bone graft. If you wish additional information concerning the fees charged for either standard or special procedures, or the office payment policy the following information is provided. Nunley DUMC - 2919 Durham, NC 27710 Important information to include when sending any request for an attending physicians statement: 1. Date symptoms first appeared. 2. Date you first saw your Duke orthopaedic surgeon. 3. Date you last worked. 4. Date you would like to return to work. - Disability forms: The length of disability from work will vary for each patient depending on the type of work you may return to and the severity of avascular necrosis. As a rule, most patients return to sedentary work, with the use of crutches at six weeks. Patients who return to positions that require walking without crutches may return at six to twelve months following surgery. Patients returning to hard manual labor return to work after one year or to some other form of work earlier. Short term disability forms for Drs. Date symptoms started. 2. Date you first saw your Duke Orthopaedist. 3. Date you last worked. 4. Date you would like to return to work. It is important to fill out your portion of the form before sending it to our office. Unless otherwise specified, the form will be sent to the address on the form.
